Vital Decisions Promotes Tracy Brubaker to Chief Clinical Officer

Vital Decisions, LLC, the leading organization in serious illness advance care planning and alignment, announced the promotion of Tracy Brubaker to Chief Clinical Officer. In this role, Tracy has direct oversight of all areas of the business that touch Vital Decisions’ clinical intervention.

As a licensed clinical social worker, Tracy has over 20 years of experience in behavioral health organizations, and has provided operational and clinical leadership across the spectrum of care including acute inpatient, residential; community based behavioral health, special education, and private practice.

Tracy joined Vital Decisions in 2015 as Director of Living Well Operations in our Cherry Hill, NJ location and since has quickly taken on additional responsibilities most recently as the Company’s Vice President of Clinical Operations.  “Tracy’s passion towards our mission and innate leadership qualities have contributed significantly to improving the care delivery of our clinical intervention.  Tracy brings a unique blend of clinical leadership and organizational change management to the role of Chief Clinical Officer,” says Leah Puccio, President of Vital Decisions.

Vital Decisions is now offering its Living Well Services through multiple modalities that include telephonic, video and, in 2019, the potential for in home service.  Tracy will continue to play an integral role in additional clinical and technology based advancements to come which will aid in Vital Decisions’ mission of being the catalyst in this country that fundamentally changes way individuals are cared for during an advanced illness.
