Living Well

Living Well is a multi-session, phone or video-conference program that helps members facing advanced illness overcome the behavioral, emotional and healthcare system barriers that prevent alignment of care with one’s values and preferences.

Those with a serious illness often feel overwhelmed by fear, anxiety and grief as their illness progresses. They worry about burdening loved ones, have concerns about differences between their wishes and family wishes, and may not have adequate communication of their care preferences with their doctors. They are uncertain about how to manage changes in health and feel a lack of control. Living Well Specialists help these individuals and their families to overcome these concerns. Our Specialists have guided over 300,000 individuals and loved ones through the Living Well Program.

How it Works

  • Individuals are identified for the program via Vital Decisions’ identification algorithms or by referral from a health plan’s care management services.
  • Over a series of discussions, our Master’s-level, Advance Care Planning Specialists use Motivational Interviewing and other behavioral science techniques to activate individuals, to explore, define, document, and communicate their personal preferences to ensure that the care they receive aligns with what matters most to them – now and in the future as their medical situation changes.
  • When appropriate, Specialists educate on palliative care, hospice and other services, and refer individuals and families.
