Care N’ Care and Vital Decisions’ Plenary Session at C-TAC Summit on Advanced Illness Care

In October, Vital Decisions attended the C-TAC ( Coalition to Transform Advanced Care) summit. During the Summit, Care N’ Care Health Plan partnered with Vital Decisions to present a plenary session titled, “Innovations in Advanced Care Alignment: Creating an Enhanced Individual and Family Experience.” Click on the link above to watch the session which features Care N’ Care’s Chief Medical Officer, Dr. David Sand and Vital Decisions’ VP of Strategic Solutions, Dr. Connie Ducaine. During their conversation they discuss innovations linked to improving individual and family experiences across the spectrum of planning for advanced care. They provided context on what it means to have a positive experience in light of an advanced illness, barriers that often exist in participating in the advance care planning process, and how these barriers can be overcome to ensure that an individual’s care aligns with their values and preferences.
