Care N’ Care and Vital Decisions Partner on Plenary Session at C-TAC Summit on Advanced Illness Care

Annually, the Coalition to Transform Advanced Care (C-TAC) brings together a diverse group of thought leaders from the payer, provider and health system space to generate solutions on how we can improve the quality of care for the millions of Americans living with an advanced illness. This year, the Summit on Advanced Illness Care took place from October 9^th – 11^th.

During the Summit, Care N’ Care Health Plan partnered with Vital Decisions to present a plenary session titled, “Innovations in Advanced Care Alignment: Creating an Enhanced Individual and Family Experience.”

The “fireside chat” session, facilitated by Dr. Ryan Van Wert, CEO of Vynca,  provided attendees with the opportunity to listen in on an engaging conversation between Care N’ Care’s Chief Medical Officer, Dr. David Sand, and Vital Decisions’ VP of Strategic Solutions, Dr. Connie Ducaine, as they discussed innovations linked to improving individual and family experiences across the spectrum of planning for advanced care. They provided context on what it means to have a positive experience in light of an advanced illness, barriers that often exist in participating in the advance care planning process, and how these barriers can be overcome to ensure that an individual’s care aligns with their values and preferences.  This dialogue also provided attendees with a context on how innovative health plans, health systems and service organizations can collaborate to provide a positive experience to member populations experiencing an advanced illness.
